Terms & Conditions

1. About us
AHC Form Generator (“we”, “us”, “our”) is a software tool operated from the United Kingdom that vet practices use to collect client and pet information and prepare Animal Health Certificates (“AHCs”). If you are a pet owner using a form linked from your vet practice, your contract for the AHC itself is with that practice, not with us — we provide the software they use.
2. The service
Our platform allows a vet practice to send you a link to submit your pet's vaccination and identity details, request vaccination confirmation from a referring vet where needed, and generate an AHC. The AHC is issued by the practice, not by us. We act as a software provider and administrative facilitator only.
We do not guarantee that an AHC will be issued. The issuing vet retains the professional and legal discretion to decline to certify where, in their clinical judgement, the animal does not meet the legal requirements for the AHC.
3. Pricing and payment
Vet practices using our platform get an initial allowance of free forms, after which a flat fee is charged per form generated, plus an optional add-on fee for automated client and referring-vet emails. These fees are charged to the practice, not to pet owners. Prices are displayed inclusive of VAT where applicable.
4. Cancellations
A practice may cancel a form at any time before it is completed. We do not process refunds for individual pet-owner bookings, since payment for our service is collected from the practice, not the pet owner — any arrangement between you and your vet practice regarding their own fees is between you and them.
5. Your responsibilities
If you are a pet owner completing a form, you are responsible for:
- Ensuring your pet's microchip was implanted before the rabies vaccination (required by EU/UK law).
- Ensuring your pet's rabies vaccination is current and valid at the time of travel.
- Providing accurate vaccination, microchip, and identity information. Inaccurate information may result in an invalid AHC and denial of entry at the border.
- Checking the specific entry requirements of your destination country, including any additional health measures (e.g. tapeworm treatment, health test certificates).
- Ensuring the AHC is used within its validity period (typically 10 days from the date of the clinical examination for most EU countries).
6. Limitation of liability
To the fullest extent permitted by law, we are not liable for:
- Refusal of entry to any country due to changes in border requirements or inaccurate information provided by you.
- Delays or failures caused by third-party services (payment processing, email delivery, etc.).
- The clinical decisions of the issuing vet.
Nothing in these terms limits liability for death or personal injury caused by negligence, fraud, or any other matter that cannot be excluded by law.
7. Intellectual property
All content on this website is owned by or licensed to us. You may not reproduce, copy, or distribute any material without our written permission.
8. Governing law
These terms are governed by the laws of England and Wales. Any disputes shall be subject to the exclusive jurisdiction of the courts of England and Wales.
9. Changes to these terms
We may update these terms from time to time. The current version is always available on this page. Continued use of the platform after changes constitutes acceptance of the updated terms.
